Job Descriptions and Personnel Files

The Company will maintain up-to-date job descriptions for all staff in our program, which will be available for inspection by DHS or the CDDP. (Please see Addendum C)

Individual personnel files will be maintained for each employee and will include (as required by OAR 411-340-0070) at a minimum:

  • Reference checks and confirmation of qualifications, obtained prior to hire.
  • Written documentation of criminal record clearance by the Department of Human Services.
  • Documentation of legal work status.
  • Documents indicating satisfactory completion of ARISE Mentors basic orientation, including instructions for mandatory abuse reporting, as well as training specific to developmental disabilities if the employee is to provide direct assistance to customers.
  • Written documentation that the employee has been notified of their status as a mandatory abuse reporter.
  • Written documentation of any substantiated abuse allegations.
  • Written documentation of any grievance filed against the staff person and the results of the grievance process, including disciplinary action, if any.
