Section 3A

Condition for Offer of Employment

  • All candidates for employment with the Company shall provide 3 personal or professional references prior to being hired.
  • All candidates for employment with the Company shall provide a qualifications statement describing why the candidate is qualified to provide mentoring services to adults with disabilities.
  • All references shall be checked and all qualifications statements will be verified prior to the Company issuing a letter offering employment.
  • All candidates for employment with the Company shall pass an approved background check completed by the Oregon Department of Human Resources in accordance with Oregon Administrative Rules (OAR) 407-007-0200 to 407-007-0370.
  • All candidates for employment will provide documentation proving legal eligibility to work in the United States.
  • All candidates for employment must be 18 years old before the start date of employment.
  • All candidates for employment must review, sign, and date a job description, including verification that the employee is capable of performing the duties described (see Addendum H).
  • All candidates for employment must disclose any founded reports of child abuse or substantiated reports.
  • All candidates must demonstrate by background, education, references, skills, and abilities that he or she is capable of safely and adequately performing all tasks assigned.
  • All candidates must have the ability and sufficient education to follow oral and written instructions and keep any records required.
  • All candidates must have the ability to communicate with customers, other staff, customer’s family members and brokerage personnel.
  • All candidates must show responsibility, maturity, and reputable character exercising sound judgment.
  • Having no substantiated abuse or neglect findings against them by DHS, CDDP or other appropriate authority.
  • Not being on the current Centers for Medicare and Medicaid Services list of excluded or debarred providers and,
  • Holding a valid driver’s license and appropriate levels of auto insurance if driving as part of job duties.
  • All employees must complete an employment application (Addendum D) and submit a resume.
