Section 5F
Unsual Incidents
The Company is required to report any unusual incident that occurs with a customer. Such incidents include, but are not limited to:
- Abuse according to current state abuse definitions
- Serious illness or injury that results in emergency hospitalization
- Death
- Acts of aggression towards a Company employee
- Involvement of police or fire departments
In the case of abuse, employees are mandatory reporters of any abuse that is observed, regardless of whether the victim is a customer of the Company, even if the alleged abuse was committed by an employee of the Company.
An incident report must be submitted for all unusual incidents using the Company’s on-line billing system. The report must be written with 5 working days of the occurrence, sooner for incidents that require an immediate response by the Company.
An employee who fails to report an unusual incident will be subject to disciplinary action that can include immediate for-cause termination.
